Overview

After many years of relative inactivity in the area of chronic illness, where often the focus has been on acute care, the challenges facing this field of health and social care are now clearly on the agenda.

Recently, there has been abundant discussion about exploring ways to improve the quality of life for clients with LTC.

This credit-bearing course, which is offered at both our Reading and West London campuses, will help you to develop enhanced competency in the management of patients with complex and changing long-term conditions (LTC).

Course requirements: Evidence of study at Level 5 (Diploma Level).

Cost and funding: This fee covers the full course cost.

  • Sept 24 - Aug 25 fees: £900
  • Sept 25 - Aug 26 fees: TBC

Credits: 20

Assessment method: Written assignment (essay) 50% and debate 50%

This course is taught on day release, select a start date to see the study days:

Study days are from 9am to 5pm.

  • Epidemiology of LTC 
  • Long-term condition policy/initiatives 
  • Assessment issues/ needs assessment 
  • Case management 
  • Effective management in specific groups eg COPD/ heart disease/diabetes 
  • Models of care eg psychosocial models 
  • Stress and illness 
  • Coping and adjustment 
  • Person centred dialogues/self-management 
  • Personalised integrated care 
  • Collaboration in management/MDT/Interagency and partnerships 
  • Mental Health Issues 
  • End of Life Care.
